Phew... where to begin. Maybe a timeline of the past couple days?
Friday
I picked up my scrip for Topamax on Friday. The neuro has me titrating up slowly, starting with 25mg daily for a week, then 25mg at night and 25mg in the morning for a week, then 50mg at night and 25mg in the morning. He hopes we can stop there, but we may have to go 50/50.
Friday night around 10PM, I took my first 25mg of Topamax (along with my standard 60mg Inderal LA). By 10:30, I had a significant case of the yawns, and by 11:00 I was asleep.
Saturday
Woke up around 7AM and had two bowls of cereal for breakfast. Where's this appetite suppressant effect? Just kidding-- I know it will take time. By 8:30, I'm spiking a migraine (hooray). A couple of Midrin do nothing. 600mg Ibuprofen does nothing. Finally hit it with a Maxalt and that does the trick. By this point, it's around noon and I am exhausted. I wind up sleeping off and on for most of the daylight hours on Saturday.
After dinner, I start feeling a bit of tightness in my chest. Normally, I wouldn't worry about it, but I had also been having some left arm pain. At about 7:30 PM, I tell the wife that I'd like to go to the Emergency Room to get checked out.
By 10PM, they decide to keep me overnight for observation.
Sunday
Good news, no heart trouble. I'm guessing the tightness in the chest was caused by the myriad of medication that was flowing through my system. By the time they release me, it's around 3:00PM. At 5:30, I spike a 4+ richter migraine without any warning. This is the first time this has ever happened to me. One second I'm fine, the next-- BAM-- headache. Since I'm still exhausted from the previous night's ordeal, I skip trying the Midrin and go straight for the Maxalt. Within 45 minutes, the pain is manageable, and within another 15, it is gone completely.
Now (Sunday Night)
It is now just past 10PM. I'm just about falling asleep as I type. The good news is that I am headache free for the moment. I think it is time to hit my Topamax and Inderal and call it a night. I can only hope that tomorrow is less eventful. I've already called off work for the day to recuperate.

Yikes, that's scary!!!! I had that happen when I got too hot with the topamax (you don't really sweat too much with it)...ended up getting a stress test and cardiac cath and the works, but this was a year into it, not days into it. I hope that the rest of your titration is MUCH less eventful and the migraines calm down SOON!!!!!
